Comprehensive initial evaluation, collaborative treatment planning, lab evaluation as needed, evidence-based medication management when appropriate, and wellness recommendations.

We respectfully request cancellations be done with at least a 24 hour notice or a fee may apply. We are understanding that emergencies may happen.

Membership may be cancelled before the next billing cycle. Benefits remain active through the end of the current billing period. Unused visits do not roll over and membership fees are non-refundable once billed.
